The US Post Office does not postmark mail with computer-generated postage.  Unless formally postmarked by the USPS, payments must be received by the last day of the applicable grace period to be considered made on a timely basis.

To expedite your payments, you can send us a signed and dated check by fax using the Check-By-Fax authorization form.  The form must be completed, signed and dated and include a copy of your completed check.  COBRA premiums paid by Check-By-Fax will be converted to an electronic payment (ACH) by electronic check conversion.


Credit Card payments (VISA or MasterCard only) are subject to a Convenience Fee of $50 for each monthly COBRA premium paid in any transaction. 

If you are making your COBRA premium payments at the end of the grace period, please be advised that your fully-completed and signed authorization form (Checking/Savings Payments or Credit Card Payments) or check copies sent by fax or other electronic method must be received within the grace period (midnight Pacific) to be to be considered having been made on a timely basis.  Invalid forms (incomplete or inaccurate) will not count as a timely payment. 


ELECTRONIC CHECK CONVERSION NOTICE 

If you send us your completed, signed check to us by mail, fax or other electronic method, you authorize us to copy your check and to use the account information from your check to make an electronic fund transfer from your account for the same amount as the check.   If you choose to make a check-by-phone transaction, we will accept your check information from you by phone.   Both the Electronic Check Conversion and the Check-By-Phone options may cause your payment to be withdrawn from your account faster than a normal check, frequently within 24 hours of its receipt.


By presenting us with a check or authorizing us to accept your check by phone, you have also agreed to pay a Non-Sufficient Funds (NSF) fee of $25 if your bank rejects or dishonors your payment due to insufficient funds.  If your payment is rejected by the bank, we may resubmit your original check amount plus the $25 fee.  If your check is rejected by your bank more than twice in any year, we reserve the right to refuse to accept checks from you and to require another form of payment.

When checks are processed electronically, your rights, remedies, and potential losses are governed by the federal Electronic Funds Transfer (EFT) Act.  For more information regarding electronic check conversion and your rights and options, we suggest the following publication:

Electronic payments from your Checking or Savings Account
by mail, fax or upload are processed with no additional fees.


To avoid paying the Courtesy fees, please mail a check.

If you use Adobe Reader, complete the form, print and
sign and then scan and post on the Secure Portal or you can fax or mail the completed forms.




COBRA Payment Requirements


When you are first offered COBRA, you (the Qualified Beneficiaries (employee and/or covered dependents)) must formally elect COBRA within sixty days (60) from the later of the date of notification or loss of coverage. 


Since COBRA is a temporary extension of existing benefits, there cannot be a lapse in coverage. This means that regardless of when you notify us of your decision to accept COBRA continuation, you are responsible for all premiums back to the day you would have lost coverage if you had not elected COBRA continuation coverage.

FIRST/INITIAL COBRA PREMIUM PAYMENT:

You must make your first payment for continuation coverage no later than 45 days after the date of your election (this is the date the Election Notice is postmarked). If you don’t make your first payment in full no later than 45 days after the date of your election, you’ll lose all continuation coverage rights under the Plan. You’re responsible for making sure that the amount of your first payment is correct. You may contact FlexToday, Inc. to confirm the correct amount of your first payment. Your coverage will not be reinstated until we receive an election form from you or your covered dependents electing continuation coverage and you pay your initial COBRA premium(s). You are responsible for making sure the amount of your initial premium payment is correct.

COBRA PREMIUM PAYMENTS ARE DUE ON THE FIRST DAY OF THE MONTH FOR THAT MONTH OF COVERAGE AND HAVE A THIRTY (30) DAY GRACE PERIOD:

After you make your first payment for COBRA coverage, you’ll have to make periodic payments for each coverage period that follows on a monthly basis. Under the Plan, each of these periodic payments for continuation coverage is due on the first day of the month for that coverage period. If you make a periodic payment on or before the first day of the coverage period to which it applies, your coverage under the Plan will continue for that coverage period without any break. The Plan will not send periodic notices of payments.

GRACE PERIOD FOR PERIODIC PAYMENTS:

Although periodic payments are due on the dates shown above, you’ll be given a grace period of 30 days after the first day of the coverage period to make each periodic payment. You’ll get continuation coverage for each coverage period as long as payment for that coverage period is made before the end of the grace period. If you don’t make a periodic payment before the end of the grace period for that coverage period, you’ll lose all rights to continuation coverage under the Plan. Your first payment and all periodic payments for continuation coverage should be made payable to FlexToday COBRA and mailed to FlexToday, PO Box 16099, Fresno, CA 93755.

 
If you are making your COBRA premium payments at the end of the grace period, please be advised that your electronic authorization form (Checking/Savings Payments or Credit Card Payments) or check copies sent by fax or other electronic method must be received within the grace period (midnight Pacific) to be to be considered having been made on a timely basis.   COBRA premiums paid by check (electronic/fax or by mail/hardcopy version) will be converted to an electronic payment (ACH) by electronic check conversion.  No additional fees apply to check conversion unless your check is returned/rejected by your bank. 


PREMIUM PAYMENT REQUIREMENTS


You will only receive an invoice and payment coupons to

  • confirm your enrollment;
  • annually at the beginning of the plan year and
  • if the cost of your COBRA premiums change.


You will not receive monthly invoices, notices or reminders.
